b'The Social Services Department, headed by Cara Willis, LSW, and supported by Jenna Tiberi, LSW, provides guidance and supportive services to residents and families both prior to and at the time of admission. Our social workers conduct thorough psychosocial evaluations for residents with consideration for their emotional, cognitive, spiritual, and social needs to ensure their best quality of life.The social workers regularly collaborate with our psychologist and provide counseling to residents and their family members, facilitate problem solving, and aid in the residents adjustment to either a short-term rehabilitation stay or a long-term move to the nursing home environment. Thorough discharge planning, which emphasizes autonomy and home safety, is S O C I A Lalso a key function of the social work team.S E R V I C E S Because Greenwood House was closed to all visitors, the social services department was key in scheduling outdoor visits for the residents and their family members and friends. Social services, in conjunction with the activity crew, also facilitated virtual visitation via social media.Upon admission, the Activities Team evaluates each resident with an individualized assessment. The purpose of each assessment is to discover the residents interests to enhance quality of life through meaningful, familiar, and enjoyable activities. The team is under the direction of Jean Stroud, ADC, Director of Life Enrichment, and consists of 10 part- and full-time activity assistants. Activities address the cognitive, spiritual, creative, social and physical interests of residents which are customized to suit each resident. Because of the pandemic and new strict Department of Health and CDC guidelines, the activity department had to completely revamp the way activities were conducted. Group activities were no longer allowed, and residents were quarantined in their rooms. The activity staff conducted hallway activities with residents in the doorways of their rooms and made one-on-one visits.
